We serve the communities of Ancaster, Dundas, Flamborough, Glanbrook, Hamilton and Stoney Creek within the City of Hamilton; and the communities of Burlington, Halton Hills, Milton and Oakville within Halton Region. The combined population of our service area is approximately 800,000 residents, of whom approximately 12,000 suffer from Alzheimer Disease or a related disorder.

OUR PROGRAMS AND SERVICES
The Alzheimer Society provides information, supportive counselling, and education to individuals with dementia, their care partners, and family members in person, over the phone, by email and in discussion groups. The Alzheimer Society also provides information, education and consultation services to community organizations and local health care agencies.

PUBLIC EDUCATION First Link Series - First Link™ offers five separate progressive learning series and covers every step of the dementia journey
| Public Education Seminars - on topics related to dementia are offered at various times and locations. These seminars will provide you with an opportunity to hear up-to-date information related to dementia. Come and learn and bring your questions! Speakers Bureau Presentations - The Alzheimer Society provides presentations to meet the needs of a specific group or organization that is requesting more information on any topic related to Alzheimer's disease or another dementia, and ways to promote brain health. Learning opportunities for Health Care Staff - our staff will provide on-site dementia education to your staff geared towards your learning needs. FAMILY SUPPORT Return
We are able to provide supportive counselling to individuals with dementia, their care partners and/or family members. This support can come in many forms, including:
- telephone
- home visits
- in-office (one-on-one or in small groups)

DISCUSSION GROUPS Return
The Alzheimer Society offers ongoing discussion groups in order for participants to share common problems and to understand that "you are not alone". People are welcome to these meetings at any time but are encouraged to attend education sessions first. We offer a discussion group to persons in the early stages of Alzheimer's disease or another dementia. Those in the early stages are encouraged to express their feelings and to share their thoughts with others that are in a similar situation

SAFELY HOME Return
This program is designed to help find the person with Alzheimer's disease or another dementia who is lost and assist in a safe return. After filling out the registration forms, this vital information about the individual is stored confidentially on a police database. The information can be accessed by police anywhere in Canada. For a one time fee of $35, an identification bracelet, a Caregiver Handbook and ID cards are provided.

PSYCHOGERIATRIC RESOURCE CONSULTANT SERVICES Return Assist and advise staff in the use of assessment tools and the development of care plans and interventions for persons with behavioural issues.
The Psychogeriatric Resource Consultant has a strong knowledge base of the management of mental illness and dementias, including behaviour management. Their skills as a facilitator and educator can assist frontline providers in dealing with specific care issues using current best practice methods.
CIRCLE OF FRIENDS Return Circle of Friends is a gathering of family and friends whose combined efforts support an individual with long-term illness, memory loss or suffering from isolation. This program assists an individual to maintain their mental, physical and social well-being. It provides a way for people living at home, in Hamilton and Halton, to stay connected to their communities. It also helps free time for the primary care partner. FRIENDLY VISITING/RESPITE Return
The Volunteer Respite Visiting Program is a free, in-home support program. Through the program, a trained volunteer provides companionship to a person with dementia through weekly visits. The volunteer can help ease the stress of caregiving and offer a warm and friendly relationship for the person with dementia and the caregiver. Each volunteer must obtain a police check and provide references to the Coordinator. The volunteer must also complete a free training session before the match begins with a client. The program is open to people living in Hamilton who are caring for someone with dementia. A counsellor meets the person with dementia and the caregiver in the home to gather information for a good match with a volunteer.n appropriate volunteer is selected and a first visit is scheduled with the counsellor. Ongoing match satisfaction is monitored and additional volunteer training/support is offered by the Society.

Resource Centre - Books, videos, and DVDs on Alzheimer's disease and other dementia topics such as coping techniques and strategies, activities for the Alzheimer person, or placement issues are available to help both family and professional caregivers. Fact sheets, Alzheimer journals and other reference materials are also available.

- Information and support in English, French or other language with the help of an interpreter
- Print material and audio visual resources in English, French or other languages
- Assistance in starting a support group within a diverse community with the assistance of a trained volunteer able to provide interpretation if required
