FOR: A self-management program for people living with dementia and their care partners.
SESSIONS: 8 weeks
PURPOSE: The program is designed to support both the person diagnosed with dementia and the person who supports them.
TOPICS: Communication; Adapting to Change; Safety, Independence & Decision-Making; finding Meaning & Purpose; Emotional Wellness; Building & Keeping Connections.
